Are Generic Drugs Actually the Same as Name Brand Ones?

Things that are simple when you’re healthy can feel nearly impossible when you’re sick — including trying to figure out which medicine you should buy to help you feel better. They all come in similar boxes and promise to do similar things, so why is this one $6 and that one $15? If you’re paying more for medication, you must be getting something more effective, right?

The short answer is that the medications with the unfamiliar names, bland packaging, and lower price tags are generics. Doug Hirsch, co-founder and co-CEO of GoodRx, a comparison website that finds the lowest local prices for medications, tells us: “The brand version of a drug had a patent preventing generic copies. But after the patent runs out, other manufacturers can begin to sell their generic versions, and that’s when the price goes down.” The US Food and Drug Administration (FDA) ensures that generic drugs “use the same active ingredients as brand-name medicines and work the same way, so they have the same risks and benefits.”

So generic medicines contain the same active ingredients and have to go through rigorous testing to prove they work in the same way, but they cost much less? Is this too good to be true? Find out in this far-from-generic explanation.

The Designer Handbag Effect

Given what we’ve just learned about generics, it seems surprising that anyone still prefers brand-name medicines. And indeed, fewer people are: A study published in 2013 in the Social Science Research Network Electronic Journal found that 70 percent of consumers without medical training bought generic headache medication over a branded option. This seems high until you learn that 90 percent of pharmacists chose generics. Caitlin Hoff, a Health and Safety Investigator with ConsumerSafety.org, an organization that keeps consumers up to date with health and safety news, believes that this partly goes back to the price tag. “Some people perceive generic medications the same way you might think of a knock-off designer good; it might be close to the original, but it’s never quite as good,” she explains. “That’s not the case when it comes to medications. To be an approved generic medication, it has to have the same API (active pharmaceutical ingredient) as the branded version, and the makers have to prove its effectiveness. So doctors know that a generic medication will work just as well for a fraction of the cost.”

What’s in a Brand Name?

It’s not only a higher price that convinces people to trust brands over generics: Seeing a familiar name is reassuring when you’re sick. Dr. Cole Sloan, PharmD, BCPS, a clinical pharmacist at the Emergency Department at University of Utah Health Care, says, “When you’re feeling ill, you don’t want to take a medication that you aren’t sure is the right stuff. When non-medical people are in pain, they tend to think of Tylenol, and when you’re focused on your pain, the generic name ‘acetaminophen’ isn’t as familiar or appealing.” The brands know this, of course. “Notice how much easier the brand names are to say and therefore remember,” Sloan adds. “Usually brands have fewer syllables than generic names.”

Getting us familiar with a brand’s name starts with marketing efforts by the company selling the drugs, but Kelly Cawcutt, MD, assistant professor of medicine at the University of Nebraska Medical Center, adds that sometimes physicians who are trying to simplify things for their worried patients accidentally compound the effect. “If I say ‘acetaminophen,’ and the patient doesn’t recognize this name, I would let them know that this is the same as Tylenol — and suddenly that’s the household name everyone knows.”

Are Brand-Name Drugs Ever Better?

There are a few specific situations where choosing the pricier drug could be better for you. “FDA regulations apply to concentrations of active drug in the body,” Sloan points out, “so sometimes generic drugs will be formulated with different inactive ingredients that some patients may not tolerate. For example, it may not be worth it for generic companies to use the same food dyes as branded products, so patients with dye allergies might have to stick to other manufacturers and ultimately have to pay more to get the brand-name drugs.” Alternatively, Hoff says you might need to stick to brands “if you are prescribed an NTI (narrow therapeutic index) drug, for example a blood thinner, which holds some risk based on its dosage. Generic medications may have a slightly different concentration, and therefore your doctor might recommend the brand instead.”

It might also be that some patients simply respond better to different formulas. “Some brand formulations may not be the same as the generic, for example tablet vs. gel capsule, which may make some difference in the non-active compounds and perceived impact,” Cawcutt says. And sometimes it’s just preference: “If you ask me what to buy, I will always strive for generic options, but if a patient swears that Tylenol works better for them than a generic acetaminophen and therefore they want to spend the extra money on it, I do not advise them to change.”

What’s the Best Way to Save Money on Medication?

The good news is that your doctor is probably on your side. “I would say that most doctors know that if they prescribe a drug their patient can’t afford, they’re not doing their job, so they do their best to make sure their patients are getting the cheapest, most effective option,” Hirsch reassures us. Cawcutt confirms this: “I routinely buy generic medications for myself and my family, and I prescribe them to my patients as long as a generic formulation is available. I think part of my job as a physician is to advocate for cost-effective medicines.” And even if you don’t have a proactively thoughtful doctor like Cawcutt, Hirsch adds, “most insurers have a ‘dispense as generic’ rule that requires the pharmacist to use the generic version when it’s available.”

Since GoodRx aims to find users’ medicines at their lowest prices, it figures that Hirsch would have some good advice on saving money: “Know that you can shop around for the best price on medications. Even if you have insurance, check the price of the drug. If you’ve got a high deductible or a steep copay, it can often be cheaper to skip paying with insurance and pay with a discount coupon instead. Also, be aware of pharmacy membership programs that can make your prescription cheaper, especially if it’s a regular prescription you’re filling several times a year. Ask your pharmacist: They often have great ideas on how to save. Try manufacturer coupons, which help make brand drugs more affordable, or ask for a longer prescription (e.g., 90 days instead of 30 days), which can make it a lot cheaper with or without a coupon.”

“Drug prices are a huge problem in this country, but people are fighting every day to make them more affordable,” Hirsch promises. Whatever brand your handbag is, if you do your best to keep your drugs generic, shopping for medication doesn’t have to be a headache on top of your headache.

I think we went down that road enough that now we’re going to pull back and reposition so we can have them all in scenes together without [Morgan] being like, a full homewrecker. Even if you weren’t exactly in line for the Olympic gymnastics team as a kid (another reason Simone Biles is so impressive, honestly), chances are, you look back on those days as ones of your peak flexibility. Like running, remembering things, and making new friends, bending gradually gets harder as you age. Fortunately, there are some things you can do to keep your flex! Here’s why you lose your bendiness — and how to get it back.


Why Your Flexibility Stalls

As though going through puberty weren’t enough of a struggle, it’s also the time when you start to lose your natural flexibility. “No one is more flexible in their life than they are as a child,” says Dr. Leython Williams, a doctor of physical therapy at Athletico Physical Therapy. “However, during puberty, children often become temporarily less flexible. Some children have slow growth spurts, while others grow very quickly. This temporary decrease in muscle length/flexibility is a result of their bones growing faster than their muscles and tendons can stretch.”

Note that Dr. Williams refers to this particular loss of flexibility as temporary — meaning we can’t blame puberty for the fact our toes seem to be getting farther away every time we try to touch them.

“There is no specific age when we wake up and our flexibility has ‘officially declined.’ However, it is a fact that as we age our flexibility decreases, which leads to decreased functional ability and mobility,” he explains. “As our bodies get older, we lose a small amount of flexibility as a result of the normal aging processes. There is loss of water in our tissues and intervertebral discs, increased stiffness in our joints, and a loss of elasticity in muscles and tendons. In our 20s and 30s, it is important that we develop a consistent static and dynamic stretching regimen to establish and maintain flexibility and range of motion more easily into our older years.”

That’s right: What you do now matters later. And this isn’t just about keeping up in yoga, Dr. Williams cautions. “It becomes imperative that we stretch regularly to decrease our likelihood of various injuries.” Jill Belland, co-founder of Barre Belle, agrees. “It’s easy to feel invincible if you’re young and injury free! Flexibility will reduce your risk of injury: What doesn’t bend will break. Increased range of motion, balance, and mobility are all linked to flexibility and contribute to overall strength and fitness.”

How to Get It Back

When you’re a kid, flexibility comes naturally; when you’re an adult, you have to take responsibility for improving and maintaining it. “Peak flexibility age for adults can differ person to person and depends largely on their physical activity habits,” says Dr. Rachelle Reed, PhD, Pure Barre’s manager of training development and barre kinesiologist. “Notably, flexibility can be improved at any age when flexibility training is incorporated into a regular exercise routine. It’s important for adults to be strategic about including flexibility training into their workouts, because maintaining flexibility and physical function as we move into middle and older adulthood is associated with better quality of life and independent living.”

There’s no one flexibility standard everyone has to meet, Belland reassures: “There is no golden rule for what perfect flexibility is person to person, but there are simple markers such as touching your toes from a forward fold for hamstrings/lower back, or reaching for your hands behind your back, with one hand behind the head and the other behind the waist, for shoulder mobility. You don’t need to be a contortionist; moving around with fluidity in your everyday life and having good range of motion in your favorite activities without feeling pain or tension is a better way to gauge your own personal goals.”

But where do you start if you want to improve your flexibility? Dr. Reed explains, “There are two main types of flexibility training that you should aim to incorporate into your routine: Dynamic stretching, where a stretch is performed by moving through a challenging but attainable range of motion several times in a row, and static stretching, where a stretch is held in a challenging but attainable position for a longer period of time (at least 10-30 seconds).”

How do you know how far to go? “Our muscles have a natural stretch reflex that prevents us from going too far into a stretch, which can also cause injury,” Belland tells us. “Aim to gently stretch through this reflex. The key to increasing flexibility is about time under tension: It takes around 20 seconds to ease through the stretch reflex. Reintroduce the stretch and hold it for 10 seconds longer, working up to one minute. The second time you introduce a stretch, the stretch reflex will diminish, allowing your body to get deeper into your stretch and also building better muscle memory.”

If you’re still worried about injuring yourself — especially if you’ve been injured in the past — Dr. Williams recommends seeing a physical therapist for guided stretches. Belland reminds us that you don’t have to go to the gym or spend hours on stretching. “Stretch a little bit every day when you can fit it in after a workout or in front of the TV. If you just don’t think you’ll stretch on your own, getting to a yoga or a barre class is helpful if you need the accountability of a dedicated time and space.”

Speaking of the gym, just as your phys ed teacher told you, stretching is especially important before and after a workout. “Far too often active individuals begin their workouts without a dynamic stretch,” Dr. Williams says. “Dynamic stretching increases one’s range of movement and blood flow to soft tissues prior to physical exertion and/or sport performance. This type of stretching is vital in improving performance and reducing the risk for injury.”

But you don’t have to be an athlete to benefit — in fact, stretching is an important antidote to all that sitting those of us with desk-based jobs tend to do. “Sitting at your desk all day can lead to decreased flexibility in hip flexors and hamstrings by keeping them in a shortened position for a prolonged period of time,” Dr. Williams warns. “Consider a standing desk or work station or taking several breaks throughout your work day to stand up and stretch.”

Instead of looking back on your kid-level flexibility as a long-ago dream, mix in a little bit of stretching into your new daily routine — especially if you work at a desk! I leave the house without makeup almost every day, so I don't know why it was so surprising to see Drew Barrymore (49) and Food Network's Valerie Bertinelli (64) appear on The Drew Barrymore Show sans makeup alongside Pamela Anderson (57), who's gone makeup-free since 2022!

"You know, and that’s when I went home into my garden and started planting things and getting into nature and going back to the trees that knew me since birth. I bought my grandmother’s property and renovated it, and I just started taking it all back. And then started peeking out without makeup. And then I started realizing, ‘Oh, I feel great as me.'"

Does Drew Barrymore have extensions in her hair?

Her honesty then inspired Drew Barrymore to take out her own extensions and get real about how perimenopause has affected her hair! "Another awesome symptom of perimenopause is you start to lose your hair," she said, throwing her extensions on the ground. "You can have a full face of makeup, feel like the makeup is beautiful, but if you're having a bad hair day, it's like, 'Nah, not gonna happen," Valerie said, while Drew agreed, "And if you are having a great hair day, you feel a lot more confident about wearing less makeup because you feel so good in your presentation that everything else sort of falls into place."

The moment was not only received super well by the internet, but it made us all emotional, too! "I’m in tears," one user commented on the TikTok. "Women deserve to be able to live authentically. We are beautiful in all forms."

"Gaahh it feels good too, to watch famous women on tv AND THEY LOOK LIKE NORMAL WOMEN!" another agrees. "I ditched makeup for quarantine and will never go back to daily wear."

This isn't the first time we've see our favorite A-listers without makeup — Lady Gaga performed at the 2023 Oscars with ripped jeans and bare skin — and we're sure it won't be the last.
